If the doctor is aware of it, they will generally not give one. My cousin was an addict and had to recover from a surgery on over the counter pain killers because the doctor would not give a narcotic.
Drug problem in United States is not going away
I've seen people go to the doctor for a bad back, get a few pills and a few months later, they are committing armed robberies to feed their new addiction. There is an army of dedicated people trying to solve the problem. They have an impossible job.
